This form captures the foundational information we need to set up your workspace, provision access, and align you with the Vlyt mission from day one. All submissions are encrypted and handled under the Mutual NDA you have already executed.

Required for HR records and campus or field operations where we need to reach a trusted contact on your behalf.

Financial Disbursement

Provide the bank account details to be used for disbursements. The primary account is compulsory. A secondary account is optional but recommended as a backup.

Primary Account (Compulsory)

Must be exactly 10 digits (NUBAN format).

Secondary Account (Optional)

Recommended as a backup disbursement channel.

Signed Documents

Upload your executed copies of the Mutual NDA and Offer of Partnership. Both documents must carry a wet or digital signature before you proceed.
